I just installed ClamWin on a fresh XP Home install with nothing but Firefox installed. The scan function works, but the right-click menu item (Explorer shell extension) "Scan with ClamWin Free Antivirus" hasn't appeared, even after a reboot. Have I missed the configuration option for turning it on?

There is an option to integrate ClamWin with Windows Explorer during installation. You have to check a box. Looks like you missed it. Just re-install ClamWin and watch for the option.

I know what it was. I saw the installation option, and read "Internet Explorer." I was wondering why there wasn't a similar option for Firefox.

It is neither an Internet Explorer nor a Fire Fox option--it's a Windows Explorer (the local machine file browser) option and isn't browser-related.
Since ClamWin is an on-demand scanner, it should be used as a backup to a real-time (on-access) scanner. Avira AntiVir, Avast, AVG and Microsoft Security Essentials are all decent real-time scanners with free versions available. The Clam Sentinel program (a separate project from ClamWin) enables you to scan in real-time using ClamWin, and it also has its own heuristic scanning engine to identify a large portion of new malware for which there are no signatures yet.
