Not everyone likes real-time scanning...
It is a constant performance barrier for specific apps that often access the HDD, such a photoshop and games.
Personally I like NO anti-virus, or an anti-virus such as clamwin or a non-real-time anti virus ( clamwin doesn't eat your resources. )
Some how I don't think clamwin are looking at real-time scanning yet. :P
Check out winpooch, it implements clamwin into real-time.

It will be coming with the new interface/ui apparently.
The current ui is coded in python, which is unsuitable for real-time scanning.
Come on the #clamwin IRC chan on Freenode to ask development questions.
