yes it's the ole2 unpacker that tries to unpack cab files that have same signature, the error is due to a fix I added to the clamav win32 port that avoid to unpack same file name overwriting the old one, since the names are almost random (the unpacker used is not capable to unpack this kind of files) there are a lot of filename clashes so I preferred to give an error instead of silently overwriting files because it's a potential security ole, think about two following files with same name in a ole2 container, the first one is a virus then second not, the second will silently overwrite the previous that is a virus and virus is not detected.
Btw since the ole2 unpacker is not able to unpack cab (yes clamav first tries to pass it to cab unpacker and it fails since installshield cabs are not supported) then it pass to ole2 and it reads bad names. Unfortunately there is no way to distinguish ole2 container from cab files (the have same signature) the is no simple way to avoid it. there is already an error handling there, by default clamav would silently overwrite files, I can also remove the error message but I think it's better to known that not known what's happening

I just downloaded the latest version of ClamWin virus scanner and launched it. It ran for 6 hours. I finally stopped it as all the messages except the Firefox Phishing message that others have reported, started with this ERROR: ... message. What do I do?

Are we sure there's no way to distinguish them? How does the operating system unpack them when installing them? Would they be only found in certain directories and not in others? Would they be used in conjunction with certain other file types that would be in the same directory? What about file sizes--would they be significantly larger or smaller than CAB files? Is there any way to make an intelligent guess as to them being OLE2?
