default Clam Sentinel system registry startup entry is hkcu\software\microsoft\windows\currentversion\run.
if i have several terminal users that work simultaniously from time to time
is it necesary to start clamsentinel.exe via their hkcu\...\run sections to enable real-time protection?
or started once in one of users sessions it should check all file-disk operations, should it?
if the previous is "yes, it should", it is good idea to change hkcu\...\run to hklm\...\run, isnt it?

It might be better to ask this on the Clam Sentinel web site. Sentinel is a separate project from Clam Win, located at https://sourceforge.net/projects/clamsentinel/ on the web. The new Sentinel version 1.13 has better multiuser support.
