As far as I know, the on-access feature is on hold, and I don't believe anything substantial has come of it. Funds are available, but the problem is getting a good programmer to devote time to the project.
If you want on-access with ClamWin, the best you can do for the foreseeable future is get the little ClamSentinel program from Andrea Russo of Italy that scans with ClamWin whenever files are added, modified, or copied on your computer. Sentinel has a decent set of heuristics that can detect new malware for which there is no ClamWin signature. Detection is adequate, but speed can be a bit slow against some malware, so I recommend using Sentinel with Malwarebytes free, Trend Micro House Call, or Dr. Web's CureIT. Sentinel is available at https://sourceforge.net/projects/clamsentinel/ on the web. ClamWin must be installed before you install Sentinel. New users should read the Simple Guide and Extra Protection article that come with the installation package.
