I've been looking for an AV app that is only on-demand (eg through the context menu) with nothing in the background.
* I don't see why clamtray must run at startup. I disabled starting with Windows and to get regular updates I used Windows scheduler to start clamtray once a day and kill the task after 10 minutes. If clamtray is configures to download the updates the moment it starts, it does the job automatically. This way I didn't have to mess with doing the updating manually or with editing config files. Easily done, but I don't see why ClamWin doesn't do this automatically.
* Compared to other AV software, Clamwin is very slow in scanning folders/whole systems -- although to be fair, it is configured to scan a lot more files by default. Still, it took 12 hours to scan my system. Easily selected profiles (e.g. scan infectable files only, like in AVG) would help, but it would still be a lot slower.
* It takes too long to start when you ask for on-demand scanning. Then it takes even longer to complete the scan. It even seems to be relatively slow decompressing zip, rar etc. This means using it with eg email or download acceleration software as a pseudo-on-demand scanner is not a good idea.
* Lots of false positives. I checked what it found with another 3-4 AV apps and they found the suspicious files OK. Not sure who to trust. Interestingly enough, these files were typically files people on the net have been suspecting (e.g. eRightSoft\SUPER files), but concerns seem to be based on rumours. Other AV apps clear these files.
* It doesn't seem to warn about inaccessible, password-protected, skipped etc files.
* It can't pause a scan or suspend it for later. This is crucial if a scan can take 12 hours!
I can go on, but these are the major issues that stand out for me.

I second the wish for having the ability to [and default to] minimal user intervention required for updates. Just do it for me
