Should I be concerned, how can I deal with it?

Files/programs that are in use at the same time ClamWin is scanning your computer can't be opened by ClamWin. this is normal. These are system files and files for other applications/processes currently in use, and you usually don't have to worry about them. You will notice that it is the same files all the time.
